Before submitting this request form for reimbursement, please be sure the following information is on file in our District Office:

1. Request for Preapproval of Advanced Training Experience form, submitted prior to enrollment and signed by the Superintendent
2. Official transcript(s) from accredited institution showing grade and credit received, or evidence of CEU courses/programs if from non-accredited institution (1/CEU for each 10 hours of contact - See Collective Bargaining Agreement for explanation)
